Use the Ohio Cremation & Memorial Society’s Arrangement Form for the State of Ohio to provide complete information about what will be included on the death certificate.

One of the last gifts we can give to our loved one’s after their death, is to honor them with a meaningful life story that details who they were, what they’ve done, and why they mattered so much to us. Most refer to this as an obituary, and this is something that will be become a family keepsake that is passed down from generation to generation to help your family know and remember your heritage. This is an easy to follow “Obituary Worksheet” that will help you, step by step, to tell the story of your loved one’s life. Once you have completed the form, submit it to one of our caring staff for us to put in our online obituary section on our website. Also, we will be glad to submit your loved one’s obituary to any newspaper you wish (fees from each newspaper will apply), or you are more than welcome to post it to your own Facebook page.

Cremation for Catholics

Cremation is still unfamiliar and emotionally uncomfortable for some Catholics. However, changes made by the Catholic Church have enabled cremation to be considered an acceptable option. Ohio Cremation & Memorial Society can provide expert assistance in cremation options for Catholics.



Caring for the Caregiver

Each month, Ohio Cremation & Memorial Society honors a caregiver from nominations received from peers, friends, and others who feel a particular caregiver excels in his or her profession. Each honoree receives recognition and a special gift. Use our nomination form for your favorite caregiver.



Flag Retirement

Ohio Cremation & Memorial Society accepts U.S. flags that are no longer worthy of display. These flags will honor a fallen veteran by being draped across them at the time of cremation. There is no charge for this service.
