When a passing occurs:

The State of Ohio requires that this Authorization Form be completed and signed prior to the cremation. Please read it carefully and ask us any questions you may have.

Pre-planning arrangements:

Each month, Ohio Cremation & Memorial Society honors a caregiver from nominations received from peers, friends, and others who feel a particular caregiver excels in his or her profession. Each honoree receives recognition and a special gift. Use our nomination form for your favorite caregiver.
Ohio Cremation & Memorial Society accepts U.S. flags that are no longer worthy of display. These flags will honor a fallen veteran by being draped across them at the time of cremation. There is no charge for this service.
